Provide initial intake information to prepare for a telehealth visit with a clinician, supporting assessment, promotion, maintenance, and restoration of patient health and wellness.
Education: High School diploma or equivalent.
Language: Proficiency in English and Spanish.
Experience: Prior customer service experience required.
Knowledge, Skills, and Abilities: Strong verbal communication skills.
