Compass Palliative Care, a California-based palliative care provider, seeks outstanding California-based Nurse Practitioners and Physician Assistants with experience and/or interest in palliative care for telemedicine appointments post-hospital discharge. The program supports flexible days and hours, and providers must reside in California.

Expected hours: 30 per week. The practitioner will be assigned a patient load and will document diagnoses, manage care, and educate patients and families,

Competenze

• CA-licensed NP or PA with eligibility to provide palliative care.
• Experience or interest in palliative care and telemedicine.
• Excellent communication and patient education skills.

Mansioni

Compass Palliative, a leading California-based palliative care provider, seeks outstanding California-based Nurse Practitioners/ PAs with experience and/or interest in palliative care. Our practice provides palliative telemedicine appointments for California-basedpatientspost-hospital discharge.

We are pleased to offer FLEXIBLE DAYS and HOURS. Provider must be residing in CALIFORNIA.

Expected hours: 30

The practitioner will be assigned a specific patientloadfor which they will be responsible. Our program is run by experienced and board-certified physicians who take pride in providing training and in-service education to their staff. They believe in the advancement of their staff as well as creating the best possible outcomes for the patients they serve.

Responsibilities include, but are not limited to:

• Assess patients in atelemedicine environment
• Provide palliative careto treat the patient best
• Define and document patient diagnoses
• Assure all patients are completely satisfied
• Patient and family education and management
• Notate findings and treatment course in patient charts, including follow-up notes
• Activelyparticipatesin departmental activities to improve training methods and achieve company goals

Requirements:

• Must be licensed as an NP or PA in California
