Qualifications

• Master's degree or higher in nursing (e.g., MSN/AGNP).
• Telemedicine care experience of 1+ year.
• Full proficiency in speaking, reading, and writing in Spanish.

Responsibilities

• Provide clinical care and ongoing management through telehealth.
• Manage medically complex Medicaid/DUAL eligible populations.
• Collaborate with physicians and care team to develop care plans.
• Coordinate with patients, families, caregivers and providers.
• Identify health changes early to prevent ER visits and hospitalizations.
• Support patients through hospital admissions, discharges and transitions home.
• Participate in advance care planning and goals-of-care discussions.
• Engage in a shared clinical triage and support model.
• Support quality initiatives including documentation for HCC coding and STAR/HEDIS measures.
• Contribute to best practices and innovative home-centered care model.
