Job summary

Qualifications

Responsibilities

• Assess patient issues and determine appropriate care path during encounters.
• Provide guidance on prescription refills and clinical questions by phone or telehealth.
• Document information clearly using SBAR format during calls.
• Collaborate with clinic staff, including LPNs and CMAs, for patient care.
• Maintain professional responsibility in a highly collaborative environment.
• Perform other duties as necessary.
