Responsibilities

• Perform intake and document patient medical history.
• Conduct assessment interviews via virtual technology.
• Collaborate with bedside RNs to develop the care plan.
• Document data into EPIC Admissions Navigator.
• Provide patient, family and significant other education.
• Maintain HIPAA confidentiality.
• Provide clinical leadership and lifelong learning.
• Perform other duties as assigned.
