Aufgaben

• Assess patient issues over the phone to determine appropriate place of care (clinic, priority care, or ED).
• Respond to patient requests for refills, results, and other clinical questions.
• Collect subjective and objective data from patient, family, or caregiver and interpret it.
• Arrange data in sequence to address anticipated or immediate patient needs.
• Utilize clinical judgement to apply decision support tools for each encounter.
• Document information in SBAR format for telephone encounters.
• Assist clinic staff by completing overdue or outstanding patient call backs.
• Support clinic LPNs and CMAs with higher level clinical questions.
