Your Health Organization is seeking a Nurse Practitioner with nephrology specialization to serve patients across South Carolina via telehealth. The role involves evaluating high-risk patients, coordinating care plans, and collaborating with a care team to improve outcomes.

Providers conduct virtual visits from home, with a full-time salary-based arrangement and potential monthly bonuses. We look for NP/PA/MD with active license, able to work flexible hours and travel between offices as needed.

Qualifications

• Nurse Practitioner license in good standing.
• Experience in home health, clinic, facility, telehealth, or specialty care preferred.
• Strong communication and collaboration within an interdisciplinary team.
• Ability to evaluate high-risk patients and develop care plans.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity of care.
• Communicate with case managers and other team members to coordinate patient care.
• Participate in team meetings to discuss progress and care coordination.
• Provide patient education and resources related to conditions and self-management.
