Third Eye Health is seeking a Nurse Practitioner to conduct comprehensive telehealth patient assessments, including history, exam, and diagnostic evaluations for post-acute care patients.

You will diagnose, treat, and manage acute and chronic conditions, prescribe medications, and collaborate with a multidisciplinary team. A NP license with relevant specialties and extensive telehealth experience are required.

Competenze

• Master's or Doctoral degree in Nursing from an accredited program.
• Current NP license with no disciplinary action; approved specialties: FNP/ACNP/ANP/AGNP.
• Primary license states: IL, MA, PA, CT, RI, MI; secondary: ME, NH, VT, IN, OH, WI, DE, MN, TX, FL, CO, MD, VA.
• Current AANP or ANCC Certification; Current CRNP License; Current NCCPA Certification; Current BLS Certification.
• Minimum of 5 years of clinical experience as a Nurse Practitioner; minimum of 3 years in post-acute care/ skilled nursing.
• Experience in telehealth or remote patient care preferred; excellent communication and detail orientation.

Our Nurse Practitioners conduct comprehensive assessments of patients via telehealth. The assessments include medical and social history, in addition to a visual, and limited physical examination. They must utilize strong clinical decision-making skills which display a solid knowledge of disease pathophysiology and pharmacology; as a clinician they will effectively manage acute conditions of post-acute patients including new admissions stabilizations.

Key Responsibilities

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program.
• Current Nurse Practitioner (NP) license free of disciplinary action; approved specialties:
• FNP, ACNP, ANP, AGNP (not all-inclusive) within the states listed below:

• Primary License states: IL, MA, PA, CT, RI, MI
• Secondary License states: ME, NH, VT, IN, OH, WI, DE, MN, TX, FL, CO, MD, VA
• Current AANP or ANCC Certification.
• Current CRNP License.
• Current NCCPA Certification.
• Current BLS Certification.
• Minimum of 5 years of clinical experience as a Nurse Practitioner.
• Minimum of 3 years of advanced practice provider experience within a post-acute care/
• skilled nursing care preferred.
• Experience in telehealth or remote patient care is preferred.
• Excellent communication and interpersonal skills.
• Strong attention to detail.
• Proficiency with telehealth technology and electronic health records (EHR) systems.
• High level of general proficiency with computers and proficiency with Microsoft Oice.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and patient-centered approach to care.
• Availability for occasional weekend and/ or evening work.

• Ability to perform the essential job functions consistently and successfully with the ADA, FMLA and other federal, state, and local standards, including meeting productivity standards

Working Conditions

• Work will be performed in a home office setting.
• Must maintain a HIPAA compliant dedicated space in the home office setting to complete documentation and to conduct telehealth visits. This space must provide privacy to protect PHI/PII and provide a professional backdrop for video conferencing.
• Access to Wi-Fi is required.
