Job summary

Third Eye Health is seeking an experienced Nurse Practitioner to perform telehealth patient assessments from a home-office setup. You will take histories, perform limited exams, diagnose, and manage acute and chronic conditions while coordinating with a multidisciplinary team.

The role requires 5+ years of NP experience, current licensure in qualifying states, and AANP/ANCC certification. Telehealth proficiency and strong communication are essential for delivering compliant, patient-centered

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program.
• Current NP license free of disciplinary action with approved specialties.
• Current AANP or ANCC Certification, CRNP and NCCPA certifications.
• Minimum of 5 years of clinical NP experience and 3 years in post-acute care.
• Experience in telehealth or remote patient care preferred.
• Strong communication, attention to detail and independence in team settings.

Job description

Our Nurse Practitioners conduct comprehensive assessments of patients via telehealth. The assessments include medical and social history, in addition to a visual, and limited physical examination. They must utilize strong clinical decision-making skills which display a solid knowledge of disease pathophysiology and pharmacology; as a clinician they will effectively manage acute conditions of post-acute patients including new admissions stabilizations.

Key Responsibilities:

• Conduct virtual patient assessments, including history taking, physical examinations, and
  diagnostic evaluations.
• Diagnose and treat acute and chronic medical conditions.
• Prescribe medications and develop treatment plans.
• Collaborate with other healthcare professionals to ensure continuity of care.
• Utilize telehealth technology effectively to deliver high-quality care.
• Maintain accurate and up-to-date patient records.
• Adhere to clinical guidelines, protocols, and regulatory requirements.
• Participation in educational programs that increase clinical knowledge base and skill set
  and demonstrated accountability for professional practices.

Qualifications:

• Master’s or Doctoral degree in Nursing from an accredited program.
• Current Nurse Practitioner (NP) license free of disciplinary action; approved specialties: FNP, ACNP, ANP, AGNP (not all-inclusive) within the states listed below:
• • Primary License states: IL, MA, PA, CT, RI, MI
  • Secondary License states: ME, NH, VT, IN, OH, WI, DE, MN, TX, FL, CO, MD, VA
• Current AANP or ANCC Certification.
• Current CRNP License.
• Current NCCPA Certification.
• Current BLS Certification.
• Minimum of 5 years of clinical experience as a Nurse Practitioner.
• Minimum of 3 years of advanced practice provider experience within a post-acute care/
• skilled nursing care preferred.
• Experience in telehealth or remote patient care is preferred.
• Excellent communication and interpersonal skills.
• Strong attention to detail.
• Proficiency with telehealth technology and electronic health records (EHR) systems.
• High level of general proficiency with computers and proficiency with Microsoft Oice.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and patient-centered approach to care.
• Availability for occasional weekend and/ or evening work.

Working Conditions:

• Work will be performed in a home office setting.
• Must maintain a HIPAA compliant dedicated space in the home office setting to complete documentation and to conduct telehealth visits. This space must provide privacy to protect PHI/PII and provide a professional backdrop for video conferencing.
