Job summary

Pod Health seeks a Care/Social Services Coordinator to support patients on our telehealth platform by linking them to essential resources and care navigation. This on-site Brooklyn role emphasizes strong communication, documentation, and collaboration with clinical teams to ensure holistic support.

The ideal candidate has 1–2 years in case management or social work, fluency in Russian, and experience with care coordination tools. HIPAA compliance and compassionate service are required.

Qualifications

• 1-2 years experience in case management, medical social work, or patient advocacy.
• Bilingual fluency in Russian.
• Healthcare/telehealth experience preferred.
• Excellent verbal, written, and virtual communication skills.

Responsibilities

• Conduct virtual assessments to identify patients’ social, emotional, and practical support needs.
• Coordinate holistic care with clinical teams (nurses, physicians, behavioral health specialists).
• Refer patients to community resources such as housing, food assistance, transportation, behavioral health, and financial navigation.
• Advocate for patients and remove barriers to care continuity.
• Document assessments, referrals, and follow-ups in the EHR or care coordination software.
• Track referrals and participate in interdisciplinary care planning.
