Overview

• Remote Triage & Long Term Care/Home Health LPN experience preferred

• Openings: 2 Part-Time positions

Available PART-TIME Schedules:

• Friday, Saturday & Sunday: 10am-6pm Central (11am-7pm Eastern)
• Friday: 4pm-8pm Central (5pm-9pm Eastern); Saturday & Sunday: 8am-2pm Central (9am-3pm Eastern)

Responsibilities

• Monitor assigned phone lines for timely responses
• Triage symptom-based calls and provide recommendations per protocol
• Assess severity, refer to appropriate care, document all actions
• Coordinate with healthcare settings and update relevant systems
• Develop and update care plans, communicate with patients via phone, text, chat, or email
• Ensure compliance, provide education, participate in quality activities, and maintain documentation

Qualifications

• Degree/certification from an accredited Nursing school
• At least one year of nursing experience
• Current LPN licensure in the state, with a compact license
• Experience with individuals with disabilities, home health, or hospice preferred

Salary Range
