Job Summary:
Provides remote consultative and supportive nursing care to critically ill patients in the Intensive Care Units (ICUs) across multiple medical centers, utilizing in-room camera and EPIC technology from a centralized hub. Collaborates with intensivists, hospitalists, MODs, nurses, respiratory therapists, and other staff to deliver timely interventions, aiming for optimal patient outcomes. Guides, coaches, and assists the clinical team in daily critical care needs according to AACN guidelines.
Essential Responsibilities:
- Performs video monitoring and assessment: Monitors ICU patient data, conducts safety oversight rounds, and collaborates with TCC physicians remotely to ensure timely consultation and intervention for ICU patients requiring immediate care.
- Responds to nursing requests: Provides real-time clinical decision support to bedside nurses during evening and night shifts for patients with urgent conditions, based on physiologic changes.
- Participates in performance improvement: Works with remote interprofessional teams to improve patient and family outcomes, leveraging technology to gather and evaluate quality metrics, and reviews critical care quality data.
- Provides clinical consultation: Offers expertise in education, assessment, and treatment of critical care patients.
- Maintains current knowledge: Stays updated with evidence-based practices and recommendations from organizations like AACN.
- Identifies improvement opportunities: Reports findings to the Physician-in-Charge, Clinical Lead, and performance teams.
- Monitors practice adherence: Ensures policies and procedures are followed, taking appropriate actions as needed.
- Participates in committees: Co-leads and engages in regional committees related to critical care.
- Serves as a subject matter expert: Provides expertise on critical care recommendations from organizations such as AACN and TJC.
- Develops sustainability strategies: Creates plans to maintain performance improvement gains over time.
- Supports policy development: Assists regional committees with P&P development, implementation, and dissemination.
