CASES is seeking a Team Leader that provides administrative and operational leadership to a multidisciplinary team of Care Managers serving individuals with complex behavioral health, housing, medical, and criminal-legal involvement needs.

Salary

The salary range for the Team Leader is $75,000 - $80,000.

Shift Hours

The Team Leader is full-time Monday through Friday 9am-5pm shift.

Flexible (for roles that have days that can be partially remote, e.g., being able to draft notes at home after going to the office/community in the morning).

What you will be doing
  • Direct and coordinate the client admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatrist. Schedule the admission interview, develop and coordinate the initial assessment and initial treatment plan; assign the most appropriate staff to the individual; and provide clinical supervision of the development of the comprehensive assessment and the treatment plan for each client
  • Participate in staff recruitment, interviewing, hiring, work assignments and orientation and performance supervision according to work rules, regulations, and policies; develop and implement staff orientation, training, and evaluation of work performance
  • Supervise Electronic Health records management assuring maintenance of the client record in compliance with OMH regulations, corporation policies, and other agency requirements. Train staff on medical record requirements; regularly review client assessments, treatment plans, and progress notes written by the staff; and supervise individual staff for medical records mastery
  • Initiate and maintain relationships, in coordination with OMH, with DOCCS, law enforcement and criminal justice systems and other health and human services agencies, and with informal community resources (i.e., landlords, employers)
  • Provide training, clinical supervision, evaluation, and professional development opportunities to staff members
  • Provide clinical oversight to the team, including person-centered treatment planning, evidence-based practices, use of service dollars, high risk/crisis incident management
  • Ensure program operates with high fidelity in accordance with the HHCM Scope of Services as per DOH/OMH
  • Serve as a visible and accessible leader who provides guidance, support, and problem-solving assistance to staff in the field and office
  • Monitor staff productivity, community-based activities, caseload management, and participant engagement to ensure quality services are delivered consistently.
  • Work with external stakeholders – such as hospitals, shelters, jails, service providers – to achieve positive client outcomes
  • Provide guidance on complex cases and crisis situations while supporting staff in making sound clinical and operational decisions
  • Lead regular team meetings focused on participant outcomes, problem solving, communication, and quality improvement initiatives
  • Other duties as required by agency executive staff
What we are looking for
  • Master’s degree in social work or related fields (human services, psychology, criminal legal)
  • At least two years of supervisory experience in a behavioral health setting
  • At least two years of experience working with adults who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal-legal involvement
  • Strong commitment to social justice and the CASES mission
  • Individuals with lived experience are encouraged
  • Proven ability to effectively manage employee performance, provide corrective feedback, and support professional growth and development
  • Strong leadership skills with experience fostering accountability while maintaining a supportive and collaborative team culture
  • Experience monitoring caseloads, participant outcomes, service delivery, and staff productivity to ensure program success
  • Demonstrated ability to utilize data and reporting tools to drive decision-making, monitor quality, and improve outcomes.
  • Strong verbal and written communication skills, including the ability to communicate expectations clearly and consistently across teams and stakeholders
Preferred Skills
  • Preferred licensed clinicians (LCSW/LMSW/LMHC) or in some limited circumstances MSW/MHC-LP with at least 1-year of relevant supervisory experience receiving license within six months
  • Experience in a multidisciplinary setting and field-based clinical work preferred
  • Fluency in Spanish a plus
  • Experience supervising Medicaid-funded care management, Health Home, ICM, or forensic behavioral health programs
  • Experience utilizing Foothold, PSYCKES, MAPP, HCS, HARP, or other care management and electronic health record systems
