This is a full-time role with after-hours coverage and a focus on person-centered care, team development, and compassionate service delivery within a home setting.

Qualifications

  • Minimum 2 years in a group home with supervisory experience.
  • Understanding of person-centered care for individuals with disabilities.
  • Excellent verbal and written communication skills.
  • Ability to stay calm under pressure and resolve conflicts.
  • Flexible schedule including evenings, weekends, and holidays.

Responsibilities

Position Overview:

We are seeking a highly motivated and compassionate individual to join our team as a Team Lead for one of our homes. The Team Lead will play a crucial role in providing guidance, support, and leadership to our staff members to ensure delivery of high-quality care and services to our individuals. This is a hands‑on, dynamic role that requires a commitment to person-centered care, collaboration, and excellence in service delivery.

Key Responsibilities:
  • Set clear objectives, define roles and responsibilities, and ensure each team member aligns with the company’s vision.
  • Communicate expectations, goals, and feedback to the team regularly and resolve any concerns or issues actively.
  • Direct assigned staff on a daily basis, ensuring quality of service.
  • Provide a variety of everyday living services and care to individuals using positive approaches, with respect for individual rights.
  • Documentation responsibilities include completing monthly and quarterly reviews, monitoring Behavior Support Plans, and making necessary updates when applicable.
  • Provide hands‑on assistance and guidance to staff in delivering person‑centered care and implementing individualized service plans.
  • Supervise and support a team of direct care staff, including training, scheduling, and performance management.
  • Ensure compliance with all applicable regulations, policies, and procedures related to the operation of the home.
  • Foster a positive and inclusive work environment that promotes teamwork, open communication, and professional growth.
  • Collaborate with other team leads, supervisors, and management to coordinate care, address challenges, and implement best practices.
  • Act as a liaison between individuals, families, staff, and external stakeholders to address concerns, resolve conflicts, and promote positive relationships.
  • Provide after‑hours and on‑call coverage as part of the direct care rotation.
  • Conduct daily audits of progress notes, vitals, MAR, ABC charts, individual funds, and controlled medication logs.
  • Prefill the medical appointment forms as required.
  • Submit Monthly LII and Weekly LII for the physical site.
  • Provide shift coverage when no one else is available.
  • Ensure RPPs (Risk Prevention Plans) are followed consistently.
  • Assist with home orientation for new staff members, ensuring smooth transitions.
  • Monitor individual medical devices and ensure they are functioning properly.
  • Complete incident reporting promptly, following up on incidents once they are documented in the progress notes.
  • Manage conflict effectively among employees, fostering a professional and supportive environment.
  • Participate in employee feedback sessions during performance management and reviews.
  • One office day every Friday to prefill medical appointments and meet with the House Manager for performance management updates.
Qualifications:
  • Minimum of 2 years of experience working in a group home or similar setting, with demonstrated leadership and supervisory experience.
  • Strong understanding of person-centered care principles and experience working with individuals with diverse needs, including developmental disabilities, and/or complex medical needs.
  • Excellent verbal and written communication skills, with strong interpersonal and organizational abilities.
  • Ability to remain calm under pressure, make sound decisions, and effectively resolve conflicts.
  • Flexibility to work evenings, weekends, and holidays as needed.
Minimum Qualifications:
  • A high school diploma or equivalent
  • One year of experience working with people with developmental disabilities.
  • Flexible schedule and availability to provide on-call coverage as needed.
  • Ability to lift between 20 and 50 lbs. and provide CPR after certification.
  • CPR, First Aid, and Medication Administration certification, a plus
  • Must be at least 18 years of age.
