Job summary

Moments Hospice is seeking an RN Clinical Manager/Team Director in Wisconsin to lead an interdisciplinary team of nurses, aides, social workers, chaplains, and volunteers, coordinating care with families and physicians.

You will ensure compliance with hospice standards, manage staff schedules, and develop care policies while fostering a collaborative, supportive work culture. This role emphasizes leadership, training, and on-call responsibilities.

Qualifications

• RN license with at least 3 years in healthcare; hospice experience preferred.
• Bachelor's degree strongly preferred.
• Leadership/management experience is a major plus.
• Valid driver's license required.

Responsibilities

• Lead interdisciplinary team of nurses, aides, social workers, chaplains, and volunteers.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and care team.
• Lead interdisciplinary meetings and document patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling and performance reviews; foster a positive environment.
• Act as central hub for communication with families and physicians.
• Analyze quality metrics and manage caseloads as needed.
• Participate in after-hours and on-call duties for seamless care.
• Maximize resource utilization and promote collaboration.
