Moments Hospice in Davenport, IA is seeking an RN Clinical Manager/Team Director to lead an interdisciplinary team and ensure holistic, compliant hospice care. You will coordinate with nurses, social workers, chaplains, and family members to deliver compassionate, patient-centered services.

Qualifications

• RN license with at least 3 years in healthcare, hospice preferred.
• Bachelor's degree strongly preferred.
• Leadership or management experience is a major plus.
• Valid driver's license required.

Responsibilities

• Lead an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure hospice regulations and standards are followed.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews; foster a positive work environment.
• Act as central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
