This vital role orchestrates one or more assigned functions critical to the success of our location(s). From scheduling to customer and clinical support, intake to health information data entry, and medical records management, the Team Coordinator is the backbone of organizational success. As you lend your expertise, you become the thread that binds patients, caregivers, and employees together in harmony.

Responsibilities

Scheduled hours for this position are Eastern Standard Time.

*Candidate must reside in one of the approved remote worker states: AR, AZ, CA, CO, FL, IL, IN, KY, MI, ND, NE, OH, SC, TN, TX, WA, WI*

As a Team Coordinator, your skillful coordination of location office functions enables us to provide unparalleled care. With each patient, caregiver, and employee you impact, you breathe life into our mission, ensuring that our services consistently exceed quality measures and expectations.

  • Processes approved physician orders. Tracks documentation requiring a physician’s signature using organization standard document management software and processes. Escalates per organization standard process when signatures are not returned in a timely manner.
  • Responsible for timely processing of designated reports, summaries, and coordination notes to physicians and facilities required to comply with home health and/or hospice CoPs.
  • Ensures discharged files in EMR are complete.

Qualifications
