Job summary

Agape Care Group is seeking a dedicated Team Coordinator for our Lexington Community Hospice House. This PRN role covers Saturdays and Sundays, 8am–6pm, with every other weekend schedule, focusing on coordinating daily workflow, calls, clinician visit assignments, and interdisciplinary meeting prep.

You will support the clinical leaders, manage inventory, and help ensure smooth operations so patients receive compassionate, high-quality care. Training and growth opportunities are available.

Qualifications

• Minimum of 1 year of experience in a health care setting.
• Scheduling and office management experience preferred.
• Reliable transportation.
• Ability to sit, stand, bend, move intermittently and lift at least 25 lbs.

Responsibilities

• Answer all incoming calls and manage daily workflow.
• Assign clinician visits and prepare interdisciplinary meetings.
• Maintain office inventory and medical supplies.
• Support the clinical leader(s) and ensure smooth operation of the agency so patients receive care.

We are looking for a team coordinator ready to lead and serve. As a team coordinator, you will be responsible for answering all incoming calls, processing daily workflow, assignment of clinician visits, preparing interdisciplinary meetings, managing and maintaining office inventory and medical supplies and supporting the clinical leader(s). Most importantly, you will be responsible for ensuring the smooth operation of the agency so patients can receive the care they need.
