Job summary

Agape Care Group is seeking a Team Coordinator to lead front-desk and clinical support activities in a hospice/palliative care setting. You will answer calls, manage daily workflow, and coordinate clinician visits while supporting interdisciplinary collaboration.

The role emphasizes organization, patient-focused service, and teamwork.

Qualifications

• Heart to serve patients and families.
• 1+ year experience in health care setting.
• Scheduling and office management experience preferred.
• Reliable transportation and ability to lift up to 25 lbs.

Responsibilities

• Answer incoming calls and triage as needed.
• Process daily workflow to keep operations smooth.
• Assign clinician visits and coordinate schedules.
• Prepare and facilitate interdisciplinary team meetings.
• Maintain and order office inventory and medical supplies.
• Support clinical leaders to ensure quality patient care.

We are looking for a team coordinator ready to lead and serve. As a team coordinator, you will be responsible for answering all incoming calls, processing daily workflow, assignment of clinician visits, preparing interdisciplinary meetings, managing and maintaining office inventory and medical supplies and supporting the clinical leader(s). Most importantly, you will be responsible for ensuring the smooth operation of the agency so patients can receive the care they need.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Associate degree preferred
• Experience: Minimum of 1-year experience in a health care setting. Scheduling and office manager experience preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs
