Job summary

Agape Care Group is seeking a Team Coordinator to lead operations, answer calls, manage daily workflow, and coordinate clinician visits. You will prepare interdisciplinary meetings, maintain office inventory, and support clinical leaders to ensure seamless patient care.

The role emphasizes compassionate service, teamwork, and reliable scheduling in a hospice setting with opportunities for professional development.

Qualifications

• Heart to serve patients and families with a passion for compassionate care.
• Education: Associate degree preferred.
• Experience: Minimum 1 year in a health care setting; scheduling and office manager experience preferred.
• Requires reliable transportation and ability to lift 25 lbs.

Responsibilities

We are looking for a team coordinator ready to lead and serve. As a team coordinator, you will be responsible for answering all incoming calls, processing daily workflow, assignment of clinician visits, preparing interdisciplinary meetings, managing and maintaining office inventory and medical supplies and supporting the clinical leader(s). Most importantly, you will be responsible for ensuring the smooth operation of the agency so patients can receive the care they need.
