Job summary

Healthcare Outcomes Performance Co. (HOPCo) is seeking a Team Care Coordinator in Jacksonville, Florida to manage patient visits and assist with medical procedures.

The ideal candidate should have a high school diploma or GED, a medical background, strong organizational skills, and excellent time management abilities. Responsibilities include documenting medical interactions, scheduling appointments, and assisting with in-office procedures.

Qualifications

  • High school diploma/GED or equivalent working knowledge preferred.
  • Medical background, taken an anatomy class, certified medical assistant, college degree(s) relating to anatomy/healthcare.
  • Knowledge of community health services.

Responsibilities

  • Accurately document medical visits, transcribe intake, and history.
  • Assist with in-office procedures such as aspirations and injections.
  • Call in prescriptions and refills authorized by the provider.
  • Schedule patient appointments and manage schedules.

Team Care Coordinator (Surgical)

Responsibilities
  • Accurately and thoroughly document medical visits including accompanying the provider into the patient exam room in order to transcribe intake, history, procedures, physical examinations and plan as given by the provider and patient.
  • Assist with in‑office procedures such as aspirations, injections, suture removal, etc.
  • Complete all disability forms/FMLA paperwork.
  • Call in prescriptions and refills as authorized by the provider.
  • Handle/triage all patient phone calls.
  • Schedule patient appointments and procedures.
  • Work with WC adjusters, home health agencies and nurse case managers.
  • Manage the Physician’s and PA’s surgical and clinical schedules.
  • Submit or renew physical therapy orders.
Education and Experience
  • High school diploma/GED or equivalent working knowledge preferred.
  • Medical background, taken an anatomy class, certified medical assistant, college degree(s) relating to anatomy/healthcare.
  • Knowledge of community health services.
Skills

Environmental Working Conditions
  • Normal office environment and some travel between clinics in assigned region.
  • Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.
Physical/Mental Demands
  • Involves sitting approximately 90 percent of the day, walking or standing the remainder.
  • Travel required.
