Responsibilities include ordering and interpreting labs/imaging, creating patient management plans, documenting in the EHR, coordinating care with the team, and participating in staff seminars and community health activities.

Formación

• Board-certified or in certification process
• SC state license with prescriptive authority
• 4-year accredited medical school training
• ACLS/BLS maintenance preferred

Responsabilidades

• Participate in quarterly chart reviews and health care collaborative.
• Order labs and imaging and interpret results.
• Develop comprehensive management plans and document in EHR.
• Coordinate care with health team to achieve outcomes.
• Provide after-visit summaries and patient education.
• Engage in staff meetings and continuing education.
• Assist in home visits, immunization campaigns and health fairs.
