Novant Health is seeking a Primary Care Advanced Practice Provider (APP) to join our growing team in South Carolina. This is a Nurse Practitioner or Physician Assistant role focused on comprehensive, patient-centered primary care and building long-term relationships with patients and families.

The ideal candidate will be collaborative, clinically strong, and committed to delivering high-quality care in a supportive team environment.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current NP or PA certification and South Carolina licensure/eligibility.
• Family Medicine, Internal Medicine, Adult-Gerontology, or related primary care experience preferred.

Responsibilities

• Provide comprehensive primary care to patients across the lifespan.
• Perform preventive, routine, and problem-focused evaluations.
• Diagnose and manage acute and chronic medical conditions.
• Develop individualized treatment plans and provide ongoing care management.
• Order and interpret appropriate diagnostic testing.
• Provide preventive health education, counseling, and screening.
• Coordinate care with physicians, specialists, and other members of the care team.
• Promote continuity of care and strong patient-provider relationships.
• Participate in quality improvement and population health initiatives.
• Support the continued growth and development of the primary care practice.

What We're Looking For
