Job summary

South Carolina Department of Public Health is seeking a Social Worker to provide psychosocial services for TB patients, assist with treatment adherence, and coordinate care with the TB Unit Manager and Nurse Case Manager.

Responsibilities include assessments, discharge planning, referrals to community resources, and ensuring confidentiality and accurate documentation within the Electronic Health Record. A valid SC social work license is required, and hybrid work is possible after 12 months.

Qualifications

• Bachelor's degree in social work from an accredited program.
• Licensed Independent Social Worker (LISW) or Licensed Masters Social Worker (LMSW) in SC required.
• Must possess or obtain required certifications and a valid driver's license; willingness to travel.

Responsibilities

• Provide counseling, emotional/mental health support and linkage to care for TB patients and families.
• Conduct psychosocial assessments and develop treatment and discharge plans with clients.
• Collaborate with TB Unit Manager and Nurse Case Manager to ensure treatment adherence.
• Assist with contact investigations and community resource referrals as needed.
• Administer social work duties and documentation per policy and confidentiality standards.
