Job summary

State of South Carolina’s Office of Mental Health seeks a Social Worker to support inpatient services in Columbia. You will assess, treat, discharge planning, and collaborate with a multidisciplinary team to promote patient wellness.

This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health– Inpatient Services, 220 Faison Drive, Columbia, SC 29203.

As the Social Worker, working under limited supervision, you will perform diversified social work duties for a complex caseload consisting of patients with multiple psycho-social problems including criminal charges. These duties include completing assessments, group and individual treatment, discharge planning, and related duties. Services are provided to an inpatient population that is 18 years of age and older.

Responsibilities for the Social Worker:
  • Maintain a caseload of Adult Civil patients requiring intensive psychiatric treatment by completing all documentation and clinical services related to the assigned caseload to include, but not limited to, case management, group, individual, and family counseling/treatment, treatment team coordination (as assigned), treatment planning in consultation with the multidisciplinary team and collaborate with the Transition Specialist regarding discharge/aftercare planning with established time frames and in adherence to agency standards.
  • Submit referrals to the Transition Specialist program within 14 days of patient admission. Participate in discharge planning activities and assume the major role for implementation of referrals, sharing of recommendations with the treatment team, family and involved agencies/programs to ensure continuity of care. This includes facilitation with other departments such as establishing financial platforms and other related matters. Review treatment plans with the involved parent/guardians/patients and develop plans with all parties to include aftercare recommendations. Participate in Social Work/Transition Specialist Staffing's and maintains use of the discharge checklist to ensure all requirements are met prior to discharge.
  • Coordinate with treatment team, family, patients and other collateral sources, monitor patients' treatment and provide information for treatment team to ensure patients' needs are being met. Authors treatment plans, interim updates and progress on goals and objectives. Maintain the treatment team calendar and communication between treatment team members.
  • Responsible for preparation of thorough and accurate Psychosocial Assessments and Forensic Review Board reports for assigned patients recommended for discharge. Community Disposition/Discharge Plan for patients approved for discharge, and other Clinical Assessments (including Report for Designated Examiner as assigned) as needed to facilitate treatment success and transition to the community. Reports are legible and/or entered EHR, professionally written with accurate punctuation and grammar.
  • Attend core competency training, MANDT, Infection Control, Patient Rights, HIPPA, etc.) and all required trainings as assigned. Attend monthly Social Work meetings and maintain Social Work Licensure. Maintain knowledge and enforce policies and procedures.
  • Assigned by supervisor, after at least one year of experience with the agency-will be able to serve as a field instructor or preceptor for students completing their field placement at Bryan Psychiatric Hospital. Responsible for completing appropriate paperwork as needed for the student and ensuring the student meets all educational/contractual requirements.
  • Provide hazardous weather coverage and weekend coverage as needed and/or assigned.
  • A bachelor's degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board.
  • A master's degree from a social work program accredited by the council on Social Work Education, LMSW-SC Board of Social Work Examiner's and one (1) year of practice experience at this level.
  • Bilingual abilities in English and Spanish (or another language) are a plus.
Post hire, employee must:
  • Have knowledge of Federal and State laws pertaining social services programs & CST/CR issues.
  • Have computer knowledge and skills sufficient to complete essential job duties in an efficient manner.
  • Have excellent professional judgement and be willing to participate in training.
  • Be capable of delivering social work services to a diverse and highly vulnerable population which presents with multiple psycho-social problems with little supervision of the technical aspect of Social Work.
