Job summary

Children's Receiving Home seeks a Care Coordinator for the Cedar Cottages Transitional Housing program. You will assess needs, create service plans, and coordinate referrals to promote housing stability for young adults aged 18-25.

You will supervise Peer Support Specialists and collaborate with leadership to ensure coordinated, trauma-informed care. The role requires at least 3–5 years in case management or housing services, a bachelor’s degree in a related field, and the ability to work

Qualifications

• Bachelor's degree in social work, human services, counseling, rehabilitation, business management, or related field.
• Three to five years of experience in case management, care coordination, housing navigation, transitional or supportive housing.
• Experience with young adults (18-25) and housing instability.
• Previous supervisory or team-coordination experience.

Responsibilities

• Provide care coordination and case management for Transitional Age Youth (TAY 18-25).
• Develop and monitor individualized service plans with participants.
• Coordinate referrals and resources to promote housing stability.
• Advocate for access to housing, health, employment, and education services.
• Supervise Peer Support Specialists and staff; coordinate scheduling and performance support.
• Maintain confidentiality and accurate documentation in HMIS and other systems.
• Collaborate with program leadership, housing navigators, and community partners.
