Position Summary:The Cedar Cottages Care Coordinator provides care coordination, case management, advocacy, and supportive services to Transitional Age Youth (TAY), ages 18-25, participating in the Cedar Cottages Transitional Housing and Support Program. The Care Coordinator assesses participant needs, develops and monitors individualized service plans, coordinates referrals and resources, promotes housing stability, and supports participants in achieving their goals.The Care Coordinator also provides frontline supervision to Peer Support Specialists and other assigned staff, including day-to-day direction, coaching, scheduling coordination, performance support, and program oversight in collaboration with the Program Manager.The position works collaboratively with program leadership, clinical, housing navigator, and staff, community organizations, government agencies, and service providers to ensure coordinated and responsive participant services. The Care Coordinator applies Trauma-Informed Care and Housing First principles and maintains a participant-centered, strengths-based, culturally responsive, and respectful approach.Essential Duties and Responsibilities:Participant care coordination and strengths-based case management working with young adults.Conduct initial eligibility and ongoing assessments of participant strengths, needs, risks, and service needs.Develop, implement, monitor, and evaluate individualized case plans and goals in collaboration with participants.Coordinate case management meetings, team discussions, and follow-up activities related to participant progress.Monitor progress, identify barriers and unmet needs, and coordinate appropriate interventions and supports.Advocate for participant access to housing, health, behavioral health, employment, education, and other supportive services.Build trusting, professional relationships while maintaining appropriate boundaries and confidentiality.Support participant engagement, stability, safety, and progress through timely follow-up and problem-solving.Referrals and Community PartnershipsCoordinate referrals to community resources, service providers, government agencies, nonprofit organizations, and other appropriate supports.Maintain current knowledge of community resources and services relevant to TAY.Collaborate with referral partners to support participant engagement and completion of service requirements.Identify service gaps and work with program and community partners to address unmet needs.Represent Cedar Cottages at community meetings, events, partner activities, and other external engagements as assigned.Assist with presentations and outreach regarding the Cedar Cottages program.Documentation, Data, and ComplianceComplete accurate, timely participant assessments, case notes, reports, service records, and other required documentation.Maintain participant records and required data in HMIS and other designated systems.Complete required daily, bi-weekly, monthly, and other program reporting activities.Maintain confidentiality in accordance with CRH policies and applicable laws and requirements.Ensure services and documentation comply with program policies, contractual requirements, and applicable regulatory or accrediting standards.Consistently follow and support implementation of CRH and program policies and procedures.Crisis Response and Participant SupportRespond appropriately to participant crises involving behavioral health concerns, substance use, conflict, safety concerns, or other circumstances affecting participant or community stability.Use sound judgment, de-escalation, relationship-based interventions, and available resources to address crises.Coordinate with internal staff, community partners, emergency services, and other resources when additional intervention is needed.Provide on-call telephone support to Cottage staff as assigned.Complete appropriate follow-up following crisis situations or significant participant concerns.Staff Supervision and Program OperationsProvide direct supervision, leadership, coaching, and day-to-day direction to Peer Support Specialists and other assigned staff.Assign and coordinate work responsibilities and assist with staff scheduling and coverage.Support recruitment, interviewing, onboarding, training, performance evaluations, professional development, corrective action, and termination processes in collaboration with the Program Manager and CRH leadership.Monitor staff performance and provide timely feedback, coaching, and support.Promote consistent, participant-centered service delivery, professional boundaries, teamwork, accountability, and adherence to program standards.Consult with the Program Manager regarding significant staff, participant, safety, or operational concerns.Participate in staff meetings, case consultation, program meetings, and other activities necessary for effective program operations.Residential Program SupportSupport the safe and effective operation of the Cedar Cottages residential environment in collaboration with program staff and the Program Manager.Monitor participant and residential needs and communicate concerns to appropriate staff or leadership.Assist with supplies, inventory, maintenance, repairs, and other residential needs.Coordinate with internal personnel, vendors, property representatives, and community resources as needed.Support compliance with program expectations, safety standards, and Housing First principles.Collaboration and Professional DevelopmentCollaborate with the Program Manager, leadership, clinical and training staff, Peer Support Specialists, Cedar Cottages team members, and community partners.Participate in required meetings, case conferences, trainings, and professional development activities.Maintain current knowledge of best practices and resources relevant to TAY and transitional housing.Travel to trainings, meetings, community events, partner activities, and other program-related activities as required.Demonstrate professionalism, reliability, flexibility, sound judgment, respect, and cultural responsiveness.Perform other duties as assigned.QualificationsEducation and ExperienceRequired:Bachelor's degree in social work, human services, counseling, rehabilitation, business management, or a related field from an accredited college or university; another bachelor's degree with relevant experience may be considered with CRH approval.Three to five years of experience in case management, care coordination, housing navigation, transitional housing, supportive services, social work, or a related human services setting.Experience working with young adults, preferably TAY ages 18-25.Experience working with individuals experiencing housing instability or other barriers to stability.Previous supervisory, lead, or team-coordination experience.Experience working in transitional housing, supportive housing, or similar residential programs.Experience in Enhanced Care Management (ECM) is highly desired.Knowledge, Skills and AbilitiesBuild effective, professional relationships
