Position Title

TASC Care Manager

Location

Pasquotank County (Elizabeth City, NC) and virtual services in Camden County.

Hours

40 hours/week, full time, Monday–Friday, non‑exempt.

Job Summary

Coastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to join our team. This position offers a unique opportunity to support justice‑involved individuals by providing case management and service coordination within the TASC (Treatment Accountability For Safer Communities) program.

Responsibilities

• Administer psychosocial screenings and assessments.
• Coordinate linkages to behavioral health treatment, medical, educational, and vocational services as needed.
• Provide ongoing professional care management services for the justice‑involved population.

Qualifications

• Education: Bachelor’s Degree or higher in a human services or related field.
• Certification: Preference given to individuals with North Carolina Peer Support Specialist certification and at least two years of experience, and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
• Experience: Knowledge of recovery, harm reduction, and wellness‑based concepts; may have lived experience with recovery and/or the justice system.

Required Skills

• Strong communication, organizational, and time‑management skills.
• Ability to work independently and manage a caseload effectively.
• Proficiency with the Microsoft Office Suite and similar platforms.
• Proficiency in using electronic health record systems and remote collaboration tools (Teams, Zoom, etc.).
• Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services.
