FreedomCare is seeking a qualified Registered Nurse (RN) for a hybrid role in Milwaukee, Wisconsin, focused on oversight of Personal Care Services and management of Personal Care Attendants (PCA). Hybrid role that requires fieldwork to visit patients’ homes in Milwaukee, WI, and surrounding areas, with the possibility of occasional commuting to our Milwaukee, WI office.

Payrate between $31 and $42 hourly. Hours for part‑time are 9-1 or 10-2 Monday through Friday (max 20 hours per week).

Personal Care Services is a caregiving arrangement in which a patient has a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in the patient’s home and providing a range of care options as their needs evolve. The service is designed to provide options for alternative long‑term care to people who meet nursing facility level care needs while preserving the dignity, self‑respect, and privacy of the patient.

Structured Family Caregiving is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in a home environment. The service is designed to provide options for alternative long‑term care to persons who meet nursing facility level care needs while preserving the dignity, self‑respect, and privacy of the patient.

The Registered Nurse will be a vital part of administrating and ensuring the efficient delivery of services, oversight of the Personal Care Attendants (PCA) and compliance with regulatory requirements. We are seeking an individual with a nursing background with elders or adults with disabilities.

Every Day You Will

• Ensure compliance with all contractual guidelines and federal and state regulations governing Medicaid and the Personal Care Services program.
• Ensure efficient day‑to‑day PCA operations by analyzing, monitoring, and optimizing workflows, processes, and procedures. Identify areas for improvement and implement solutions to enhance productivity.
• Provide oversight to PCS/SFC caregivers in conjunction with the Care Manager.
• Conduct initial home visits to define the PCA’s tasks and develop activity plans.
• Perform regular evaluations every 60 days to monitor the PCA’s performance, consumer satisfaction, and adherence to the activity plan.
• Respect client and employee rights and privacy, ensure the security of protected information, practice in an ethical manner, and be compliant with the agency’s compliance program and privacy policy.

Education And Experience

• Valid and active RN License from the Wisconsin Board of Nursing.
• 2 years of recent work experience in direct care of elders or adults with disabilities.

Compensation Range

Base pay: $40.00 to $42.00 per hour at the commencement of your first year of employment. Compensation decisions are dependent on skill set, years of experience, education, location, and licensure/certifications. Pay range: $31 - $42 USD.
