Job summary

Easterseals PORT Health in Clinton, NC is seeking a compassionate Tailored Care Manager to support individuals by evaluating needs, creating care plans, and coordinating services across physical and mental health, social determinants, and life challenges.

The role involves travel within Clinton, NC and surrounding counties, a full-time schedule from 9 AM to 5 PM on weekdays, and competitive benefits. QMHP credentialing eligibility and 2+ years’ experience are preferred.

Qualifications

• Bachelor’s degree in health, psychology, sociology, social work, nursing or related human services; Master’s degree preferred.
• Eligible for QMHP credentialing.
• Minimum 2 years of experience with mental health and/or substance use populations.
• Valid driver’s license, current auto insurance, and a clean driving record.

Responsibilities

• Evaluate clients’ needs and develop personalized care plans.
• Coordinate services addressing physical and mental health, social determinants of health.
• Travel throughout Clinton, NC and surrounding counties to connect clients with resources.
• Participate in rotating after-hours support.
