Saint Alphonsus Health System in Baker City, Oregon seeks a Program Coordinator for the Ortho Med Surg unit. This full-time day shift role leverages your coordinating experience to support swing bed operations and patient transitions.

You will coordinate the swing bed program under supervision, work with inpatient units, providers, therapy services, and care management to grow swing bed volume, and serve as a central contact for discharging hospitals and patients.

Qualifications

• BLS certification required at hire if not ACLS/PALS.
• Must communicate clearly in English and work with patients and teams.
• Must hold a PTA or CNA license in Oregon.

Responsibilities

• Coordinate the swing bed program under Medical/Surgical supervision.
• Collaborate with inpatient units, providers, therapy services and care management.
• Act as central contact for discharging hospitals and swing bed patients.
• Support patients and families for smooth transitions and care continuity.
