Job summary

Saint Alphonsus Health System in Baker City, Oregon seeks a Program Coordinator for our Ortho Med Surg to coordinate the swing bed program and support patient care across units. This full-time, day-shift role emphasizes teamwork, multi-tasking, effective communication, and collaboration with inpatient units, providers, therapy services, and care management.

You will serve as the central contact for discharging hospitals and patients, ensuring quality care throughout the stay and helping patients

Qualifications

• BLS for Healthcare Providers certification required at hire.
• Verbal skills to present information and ideas.
• Read, write, speak and understand English.
• Ability to multi-task and manage time.
• Oregon PTA or CNA license required.

Responsibilities

• Coordinate the swing bed program under the direction of the Medical/Surgical supervisor.
• Grow swing bed patient volume through collaboration with inpatient units, providers, therapy services, and care management across Saint Alphonsus Healthcare system and other acute care hospitals.
• Serve as a central point of contact for discharging hospitals and for patients/families in the swing bed program.
• Ensure quality care for patients of all ages across the life span and support patients throughout their stay to facilitate smooth transition back into the community.
• This position will provide clinical support and fulfill job duties as necessary within their licensed professional field, including patient care activities both direct and indirect for swing bed patients.
