Job summary

Help of Southern Nevada seeks a dedicated youth case manager to provide direct services under the supervision of the Case Manager Supervisor. You will develop client-centered plans, track progress, and facilitate group sessions for homeless and at-risk youth in the Emergency Shelter Program.

Responsibilities include maintaining accurate records, coordinating identifications, and participating in multi-agency teams. Flexibility for varied hours and strong communication skills are essential.

Qualifications

  • Bachelor degree in social work or four years of equivalent experience.
  • Knowledge of local social service agencies and programs related to homeless/at-risk youth.
  • Experience in case management, intake processes, and progress tracking.
  • Ability to communicate across diverse backgrounds and maintain boundaries.
  • Knowledge of applicable statutes, codes, and agency policies.
  • CPR certification and TB test as required.
  • Must pass background and pre-employment drug screening, and have NV driver's license.

Reports to: SWHYC Case Manager Supervisor

Purpose:

Under the direct supervision of the Case Manager Supervisor, this position provides direct case management services to homeless and at-risk youth in the Emergency Shelter Program.

Duties, Functions and Responsibilities:
  • 1. Create Client Centered Case Plan with residents that reflect the needs and desires of the individual, within 72 hours of intake.
  • 2. Monitor resident’s progress and provide follow-up services by facilitating weekly sessions to address the client’s needs and progress towards achieving goals.
  • 3. Complete monthly Client Centered Case Plan reviews with the client, and obtain signatures.
  • 4. Enter case notes in true time data (daily) into HMIS, reflecting activities of the client’s progress.
  • 5. Assist the youth in obtaining vital identification documents.
  • 6. Maintain audit ready files at all times. (Random audit checks are regularly performed)
  • 7. Maintain clear boundaries with all clients and providers.
  • 8. Complete exit summaries within 72 hours of exit from the program.
  • 9. Maintain accurate documentation of service to residents. Enter accurate resident data into computer reflecting resident activities and their progress.
  • 10. Must be able to communicate effectively with persons from a variety of social, cultural, and economic backgrounds.
  • 11. Organize and facilitate daily groups according to the group schedule. Maintain group log following the completion of the group.
  • 12. Provide direct supervision of all residents during assigned shift.
  • 13. Responsible for attending weekly supervision meetings with Program Supervisor.
  • 14. Report all program violations immediately following incident.
  • 15. Provides crisis intervention as needed.
  • 16. Completes all required paperwork and logs, including incident reports, bus pass logs, and curfew extensions.
  • 17. Participate in multi-agency, multidisciplinary team meeting to report and review resident progress.
  • 18. Provides transportation to appointments when necessary
  • 19. Attends court hearings when necessary.
  • 20. Attend training when assigned by leadership.
  • 21. Transport clients as necessary.
  • 22. Performs other duties, as assigned by leadership.
Knowledge, Skills and Abilities:
  • 1. Knowledge of local social service agencies and programs as they pertain to homeless and at risk youth.
  • 2. Knowledge of applicable statutes, ordinances and codes.
  • 3. Knowledge of social service work, interviewing/referral techniques.
  • 4. Experience and understanding of homeless at risk youth issues.
  • 5. Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
  • 6. Communicate effectively with persons from a variety of social, cultural and economic backgrounds and demonstrated understanding and sensitivity to culturally diverse populations.
  • 7. Effective written, verbal and listening skills. Independently initiates and responds to correspondence.
  • 8. Ability to maintain order and exercise appropriate judgment in crisis situations.
  • 9. Is flexible and can adjust to shifting priorities.
  • 10. Establish and maintain cooperative working relationships with residents, fellow employees, other agencies and the general public.
  • 11. Organize work schedule, and budget time.
  • 12. Understand and follow all agency policies and procedures.
  • 13. General computer knowledge and ability to use Microsoft Office software.
  • 14. Ability to work flexible hours as required by the resident case load.
Qualifications, Education and/or Equivalent Experience:
  • 1. Bachelor Degree in Social Work and/or 4 years of experience substantially equivalent to a bachelor degree, or combination of education and experience
  • 2. Professional experience as required by education
  • 3. Must have a valid Clark County Health Card
  • 4. Must pass TB test
  • 5. Must have current CPR certification, and must keep certification current at all times.
  • 6. Must pass a background check and pre-employment drug screening. Random drug screenings are performed.
  • 7. Must have a current NV driver’s license and clean DMV record. Driver History Reports will be required to be submitted on a quarterly basis.
Working Conditions:

Work is performed in an indoor/outdoor environment with driving required at times. May be subject to sitting, lifting, bending, squatting, walking, standing, pushing and pulling, simple grasping and climbing.
