SWAN Adoption Coordinator

SUMMARY

The SWAN Adoption Coordinator will be responsible for coordinating the Statewide Adoption Network (SWAN) program and Post-Permanency Services (PPS). This role includes providing case management support both before and after placement, as well as offering assistance to the adoption program unit as needed.

JOB DUTIES AND ESSENTIAL FUNCTIONS
  • Coordinate the SWAN and Post Permanency Services programs to ensure services for families and children meet all program requirements.
  • Coordinate the completion of child preparation services as referred.
  • Update and maintain accurate records for each assigned client.
  • Complete all required SWAN benchmark reporting.
  • Ensure all documentation and benchmark reports related to post-adopt programs comply with county and state contracts.
  • Coordinate the completion of child profiles as referred.
  • Coordinate the completion of family profiles as referred.
  • Coordinate the completion of finalization services as referred.
  • Manage an assigned SWAN and Post Permanency Services adoption caseload.
  • Assist the adoption program unit in building and expanding the SWAN program, ensuring department functions are carried out effectively and Gladney is appropriately represented.
  • Attend adoption-related educational events to obtain required continuing education credits.
  • Represent the agency at public affairs events, adoption committees, and intra-agency activities as required.
  • Provide statistics and reports on SWAN activities as needed.
  • Represent Gladney Adoption Associates at SWAN meetings.
  • Provide backup support to other Social Work positions as needed.
  • Perform other duties and special projects assigned to support organizational objectives and enhance operational efficiency.
QUALIFICATIONS
  • Bachelor’s degree in Social Work or a closely related field is required; a Master’s in Social Work (MSW) is strongly preferred.
  • State social work licensure is preferred and may be required depending on caseload and program needs.
  • Minimum of six months of direct experience providing SWAN adoption and family services.
  • Proven experience delivering public presentations and representing an organization in professional settings.
  • Excellent written communication skills, with the ability to prepare clear reports, case notes, and documentation.
  • Strong verbal communication skills, including the ability to communicate effectively with families, stakeholders, and partner agencies.
  • Demonstrated problem-solving abilities with sound judgment and decision-making skills.
  • Skilled in facilitating meetings and services involving families and children.
  • Comfortable with public speaking and presenting information to diverse audiences.
  • Strong assessment and analytical skills to evaluate client needs and program requirements.
  • Highly organized, with strong planning and time management skills to manage multiple cases and tasks simultaneously.
  • Proficient in basic computer applications, including word processing, email, and database management.
  • Ability to work independently as well as collaboratively within a team environment.
PREFERRED REQUIREMENTS
  • Master’s degree in Social Work (MSW) or a closely related field.
  • State social work licensure (preferred or required depending on caseload and program needs).
  • Skilled in facilitating meetings and providing services involving families and children, with strong assessment and analytical abilities.
  • Comfortable and experienced in public speaking and presenting to diverse audiences.
  • Strong organizational, planning, and time management skills with the ability to manage multiple cases effectively.
