Qualifications

• High school diploma or GED required.
• Completion of an accredited Medical Assistant program or LPN licensure required.
• Active LPN licensure in Louisiana preferred.
• Minimum 2 years of clinical experience as an MA or LPN, preferably in oncology, primary care, or chronic disease management.
• Experience with chart preparation, patient intake, and EMR documentation required.
• Prior experience in a survivorship, care coordination, or navigation setting strongly preferred.
• Familiarity with oncology workflows and multidisciplinary care teams a plus.

This individual will work directly with patients and clinical teams to deliver survivorship care—preparing charts for visits, supporting or conducting patient encounters, coordinating follow-up, and ensuring care plans are completed and communicated.
