Job summary

Qualifications

Responsibilities

• Conducts comprehensive patient assessments and notifies physicians of changes.
• Develops and evaluates individualized care plans with education and discharge planning.
• Prepares patients for procedures and administers medications safely.
• Regularly reassesses patient responses and adjusts care as needed.
• Communicates care plans and handoffs with the team for seamless transitions.
• Maintains a safe environment and contributes to quality and patient satisfaction initiatives.
