City of Hope in Thousand Oaks, CA, seeks an experienced Registered Nurse to support breast and gynecologic oncology patients through all phases of care. You will provide clinical navigation, education, and coordination with physicians and the care team, ensuring a compassionate patient journey.

The role emphasizes collaboration in an outpatient setting, triage, and family support, with a focus on high-quality outcomes and patient advocacy. California RN licensure is required.

Competenze

• Associate degree in nursing.
• Current California Registered Nurse license.
• Minimum of three (3) years experience as an RN.
• Current BLS through American Heart Association.
• Strong clinical assessment, critical thinking, and telephone triage skills (preferred).
• Experience with patient education, care coordination, and multidisciplinary collaboration (preferred).
• Oncology nursing experience, particularly in breast or gynecologic oncology (preferred).
• Experience supporting patients through surgical procedures and post‑operative recovery (preferred).
• Experience with patient navigation and coordinating complex care plans (preferred).
• Oncology Certified Nurse (OCN) certification (preferred).

Mansioni

• Provide nursing support and coordination for patients undergoing breast and gynecologic oncology surgical procedures.
• Support patients throughout the pre-operative and post-operative phases of care, including preparation, recovery guidance, and follow-up.
• Conduct telephone triage, assess patient concerns, provide clinical guidance, and coordinate appropriate follow-up with physicians and the care team.
• Facilitate new patient introductions and consultations, helping patients understand the care team, treatment process, and available resources.
• Provide ongoing patient and family education regarding diagnoses, surgical procedures, pre-operative instructions, post-operative care, and recovery expectations.
• Serve as a patient advocate and resource, helping patients navigate their cancer care journey.
• Coordinate care across physicians, advanced practice providers, ancillary services, and other members of the multidisciplinary team.
• Identify patient needs and connect individuals and families with appropriate clinical, educational, and supportive care resources.
• Monitor patient progress, address clinical concerns, and elevate issues to the appropriate provider when necessary.
• Promote continuity of care through proactive communication, follow-up, and patient engagement.
• Document patient interactions, clinical assessments, education, and care coordination activities accurately in the electronic medical record.
• Collaborate with the broader oncology team to ensure a seamless, compassionate, and high‑quality patient experience.
