City of Hope is seeking an Oncology Nurse Navigator to work on-site from 8:00a to 4:30p, Monday through Friday. The role focuses on disease management, patient education, and coordination of care for oncology patients.

You will document assessments and collaborate with the care team to support patient quality of life and treatment plans. Qualifications include 3–5 years of clinical nursing experience, IL RN license, and BLS certification.

Qualifikationen

• 3-5 year clinical nursing experience.
• Current IL RN license.
• BSN or higher preferred.
• BLS through American Heart Association.
• OCN Certification preferred.
• Bio/Chemo Certification preferred.

Aufgaben

• Triage patient issues in person or by phone and coordinate with the care team.
• Provide disease management guidance and explain treatment plans to patients and families.
• Document assessments and interventions in the EMR promptly.
• Collaborate with physicians and nurses to develop a multidisciplinary care plan.
• Educate patients using appropriate resources and document in the EMR.
