Responsible for providing leadership and direction for Discharge Planning and Transitions of Care within the acute hospital, monitoring quality indicators to assure appropriate social and transitional services are provided to patients and families. Develops and maintains relationships with physicians, nursing supervisors, payers, and community resources/agencies to provide needed services for indigent, uninsured, and underinsured populations.

Responsibilities

• Coordinate and monitor all Care Management team activities.
• Provide leadership, coaching, and mentoring to the Care Management and Social Works staff.
• Lead Discharge Planning and Transitions of Care within the acute hospital.
• Monitor quality indicators to assure appropriate social and transitional services for patients and families.

Qualifications

• Education: Associate's Degree from an accredited school of Nursing required or Master's degree in Social Work and current Social Work licensure in the State of Georgia required (Bachelor's Degree from an accredited school of Nursing preferred).
• Work Experience: 3 years of experience in care management, medical social work, or transitional care management required.
• Licenses and Certifications: RN – Registered Nurse – Georgia State Licensure and/or NLC/eNCL Multistate Licensure (Current license in the state of Georgia as a Registered Nurse or NLC/eNCL Multistate License Upon Hire Required) or LMSW – Licensed Medical Social Worker (State Licensure, Licensed Master Social Worker in the state of Georgia Upon Hire Required).
