Jobtailor seeks a qualified professional to conduct Supports Intensity Scale assessments (SIS-A) for adults and children in Virginia. You will document results and coordinate with internal teams to ensure accurate reporting.
Role requires statewide travel, occasional overnight stays, and assessments at homes, agencies, case manager offices, day programs, or remotely via Zoom or Microsoft Teams. Broadband internet at home is required.
Conduct Supports Intensity Scale–Adult Version (SIS-A) 2nd edition assessments and state supplemental questions with individuals with intellectual and developmental disabilities
Conduct Supports Intensity Scale–Children version assessments when applicable
Participate in AAIDD training and pass an IRQR following training and annually thereafter
Clearly communicate the purpose and reason for assessments
Conduct assessments timely and professionally
Complete and document assessment information consistently
Foster positive relationships and act as liaison with internal and external customers
Represent PCG and the client professionally
Travel throughout the state for assessments, with occasional overnight stays depending on schedule and geography
Conduct assessments at locations selected by the individual and their team, including homes, provider agencies, case manager offices, day support programs, residential provider locations, or virtually
Demonstrates expertise in conducting Supports Intensity Scale assessments for individuals with intellectual and developmental disabilities, while ensuring compliance with relevant laws and maintaining confidentiality of PHI. Proficient in utilizing technology for assessments and documentation, fostering positive relationships with clients and stakeholders.
