Job summary

Merakey is seeking a Supports Coordinator to join their team. The role focuses on coordinating and supporting caseloads across multiple counties in Pennsylvania, helping individuals maximize their daily lives and develop individual service plans based on assessments.

Strong communication with individuals, families, and providers is essential to ensure supports meet personal goals. The position offers a PRN schedule with a pay rate of $23.50 per hour, and Merakey provides a range of benefits to

Responsibilities

• Coordinating and supporting caseloads in Lackawanna, Susquehanna, Wayne, and/or surrounding counties in Pennsylvania.
• Assist and support the individuals in maximizing their "everyday life" by identifying individual's needs, strengths, lifetime goals, and preferences in services/supports.
• Assisting each individual in the development of their individual service plan, based on the results of assessments designed to develop outcomes appropriate to the individual's "personal vision."
• Maintain regular communication and contact with each individual in order to ensure that all aspects of needed/necessary supports are intact.

Education

Bachelor's degree with 12 social science credits

Associate degree + 2y paraprofessional experience

12 credits + 3y paraprofessional experience

12 credits + 2y professional experience in related fields

Must Include One Of The Following

• A bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science, Preferred


• Two years of college (including earning an associate degree) with 12 eligible credits AND two years’ experience in a position performing paraprofessional case management functions.


• 12 eligible credits AND three years’ experience in a position performing paraprofessional case management functions.


• A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years’ professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.
