Merakey seeks a Supports Coordinator to manage a caseload across Lackawanna, Susquehanna, and surrounding counties in Pennsylvania. You will assess needs, develop service plans, and coordinate with individuals, families, and providers to maximize daily living and community participation.

The role emphasizes Positive Approaches and Self-Determination, with flexible daytime hours and potential for evening/weekend work to meet client needs. Earn $17–$23.50 per hour as a fee-for-service position.

Qualifications

• Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social science is required.
• Two years of college with 12 credits and two years' paraprofessional case management experience required.
• Three years' paraprofessional case management experience with 12 credits also acceptable.
• Combination of 12 credits and two years' professional experience in related fields acceptable.

Responsibilities

• Develop and implement individual service plans based on assessments.
• Collaborate with individuals, families, and providers to identify needs and supports.
• Maintain regular communication to ensure all required supports are in place.

Education

Bachelor's degree with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science

Two years of college with 12 eligible credits AND two years' paraprofessional case management experience
