The Guidance Center in Michigan is seeking a full-time Supports Coordinator Developmental Disabilities to coordinate services for individuals with intellectual and developmental disabilities. You will link, monitor, advocate, and refer to ensure Person Centered Plans are created and documentation is completed according to standards.
A bachelor’s degree in a Human Services field is required along with relevant experience.

The Supports Coordinator Developmental Disabilities is responsible for ensuring all service needs of a person receiving services are met by providing linking, coordinating, monitoring, advocating, negotiating and referral services through the provision of supports coordination services. The Supports Coordinator Supports Coordinator Developmental Disabilities is responsible for the assurance of the creation of the Person Centered Plan, completing appropriate documentation including assessments other than Integrated Health Assessments, as well as monitoring of the established goals and the implementation of various treatment plans. Regular working hours are expected to be Monday through Friday 8:30 – 5:00 with four (4) hours per week after 5 p.m. Due to the needs of the person receiving services and the program these hours may be adjusted to include weekend hours.
We have a market-based compensation structure. The estimated salary for this position will vary based on a candidate's transferable work experience and qualifications. Hourly rates start at $23.82 for this role. The pay is just one component of total compensation including a comprehensive benefit package of affordable medical plans, employer paid dental, vision and long‑term disability, 401(K) with employer contribution, flexible spending accounts for medical and dependent, paid time off, 11 paid holidays, and employee assistance program.
