Job summary

Judson Center in Wayne County is seeking a Supports Coordinator for the Outpatient Children’s Program. The role focuses on coordinating services for children, linking families to community resources, and developing person-centered service plans with ongoing care coordination.

Ideal candidates have experience in mental health with children and families, strong teamwork and EHR skills, and knowledge of local resources.

Qualifications

  • Experience with mental health populations, especially developmental disabilities or mental illnesses.
  • Willingness to work with children and families in community or home settings.
  • Clinical awareness and sensitivity, including belief that individuals with developmental disabilities can participate meaningfully.
  • Knowledge of local human services resources.
  • Ability to facilitate small groups and work well in a team.
  • Competency with electronic health records and Microsoft Office.
  • Ability to comply with licensing rules, accreditation standards, ethics, reimbursement rules, and Judson Center policies.

  • Coordinates support and ancillary services for children.
  • Develops and monitors individual service and support plans with referrals for ancillary services and care coordination.

Primary Duties and Responsibilities include
  • This position facilitates consumer integration within their communities.
  • Coordinates support and ancillary services for children.
  • Develops and monitors individual service and support plans outlining support services to be provided, providing referrals for ancillary services, and care coordination to assist children and their families.
  • Demonstrate knowledge of and actively support culturally competent recovery-based practices and that are person centered.

  • Clinical awareness and sensitivity, including a belief that individuals with developmental disabilities can be meaningful participants in our community.
  • General skills and knowledge of local human service resources
  • Ability to facilitate small groups and work effectively in a team environment.
  • Demonstrated competency in computer skills using electronic health records and Microsoft Office.
  • Ability to comply with all applicable licensing rules, accreditation standards, code of ethics, insurance reimbursement rules, and Judson Center's policies and procedures.
Education, Certificates, Licenses, Registrations
  • Bachelor's degree in a mental health related field from an accredited school.
  • Valid Michigan Driver's license with a good driving record.
  • A minimum of a Bachelor's degree in a human services field and one year of experience with individuals with developmental disabilities.
