Qualifications

• OR High School diploma with three years of experience in human services.

Responsibilities

• Conduct client interviews, assess eligibility, and enroll individuals in agency programs.
• Assist clients in developing action plans to improve their circumstances and achieve self-sufficiency.
• Provide referrals and follow up with community resources to address clients’ social service and health needs.
• Collaborate with clients to create and implement personalized action plans with clear goals.
• Maintain knowledge of local social service and health agencies, fostering strong referral partnerships.
• Prepare reports and enter client data accurately into the system.
