Job summary

Episcopal SeniorLife Communities in Rochester, NY is seeking a full-time Supportive Housing Manager to oversee ESSHI services at 270 East Avenue. You will lead Service Coordinators, coordinate wellness, intake, and community outreach to ensure high-quality housing and support services.

Responsibilities include staff training, budget management, supervising Health and Wellness Coordinator and Drivers, and ensuring program compliance with funder guidelines.

Qualifications

• Associate degree minimum required in a Health and Human Service field with three (3) years’ experience providing Care Management for individuals with experience homelessness, mental illness and/or Older Adults and knowledge about community resources.
• Must have a valid New York State driver’s license with a clean driving record and willingness to drive a van to transport participants.
• Experience managing a Supportive Housing program or similar community-based program.
• Experience working with homeless populations.

Responsibilities

• Manage Service Coordinators to ensure participant service plans and health/wellness assessments are current per DoH guidelines.
• Conduct weekly high risk resident meetings with Service Coordinators.
• Provide training and orientation for newly hired staff to familiarize them with ESLC policies and procedures.
• Lead and collaborate to facilitate programming enriching ESSHI participants’ lives.
• Perform service coordination duties as needed.
• Lead, manage and supervise Health and Wellness Coordinator, Service Coordinators and Driver.
• Outreach to community referral agencies to inform of the program and obtain referrals.
• Schedule and conduct apartment tours for program enrollment.
• Intake potential residents and enroll them when appropriate.
• Work with outside vendors to coordinate services.
• Ensure participants receive quality services with high satisfaction through regular check-ins.
• Monitor and approve timecards; manage budget and grant targets.
