Job summary

The North Carolina Department of Health and Human Services (DHHS) is seeking a Human Services Planner/Evaluator I (S) to support the Housing Assessor role. This field-based position works with MFP Transition Teams, housing providers, hospitals, and managed care organizations to expand access to supportive housing for individuals with disabilities.

Responsibilities include matching clients to housing resources, managing waitlists, and guiding stakeholders on housing policies and fair housing

Qualifications

• Experience working with housing and homeless programs, identifying barriers to housing and coordinating supportive services.
• Proven ability to apply affordable housing policies, fair housing requirements, and supportive housing practices.
• Experience interpreting and applying federal/state rules for low-income housing finance and programs.
• Ability to analyze housing/referral data to support decision-making and tracking of resources.

Responsibilities

• Review and process housing referrals for affordable and accessible housing resources.
• Match available housing units, subsidies, and resources with individuals based on needs and eligibility.
• Manage and maintain waitlists and interest lists for housing resources to ensure timely placement.
• Review property vacancies and update information on units, contacts, and waitlists in tracking systems.
• Provide technical assistance and guidance on housing resources and fair housing requirements.
• Serve as a liaison between referral agencies and property management to facilitate placements and address concerns.
• Assist with conflict resolution for tenancy or housing-related challenges.
• Participate in housing team meetings and program improvement initiatives.
• Develop and support training activities for partners to understand housing policies and procedures.
• Provide ongoing technical assistance to strengthen community housing transitions.
