Southwest Network is seeking a dedicated professional to facilitate Child and Family Team meetings and assess youths using developmental tools. The role involves home visits and community outreach to support foster families and care providers.
You will collaborate with caregivers, group homes, and shelters to identify needs and link youths with essential services, ensuring holistic care.

You will facilitate or assist in the facilitation of Child and Family Team (CFT) meetings and conduct observations and assessments of youths using developmental and/or mental health assessment tools and trauma screenings. You will work in members’ homes and out in the community, maintaining regular contact with foster families, caregivers, group homes and shelters to assess the needs of assigned youths and assist youths/families in securing needed services.
