Job summary

Pathfinder is seeking a Supported Living Assistant Manager in Plymouth, IN. You will supervise DSPs, coordinate care, and support daily operations to ensure safe, person-centered supports in residents' homes.

You will lead team meetings, monitor training compliance, and collaborate with families and leadership to ensure shift coverage and high-quality services. A high school diploma and driver’s license are required.

Qualifications

• High school diploma or GED required.
• Minimum 2 years of experience in human services.
• Must be at least 18 years old.
• Ability to communicate clearly in English.
• Valid driver’s license with a safe driving record.

Responsibilities

• Provide direct oversight and support to Direct Support Professionals (DSPs) across assigned residential sites.
• Coordinate medical appointments, day program participation, and transportation needs; support PCISPs.
• Maintain accurate case notes, Medicaid Waiver documentation, and billing records.
• Ensure homes meet safety, health, and quality standards and manage medication documentation.
