Job summary

Jobtailor is seeking a dedicated Support Coordination professional in New Jersey to assess individual needs using the NJCAT and develop comprehensive Individualized Service Plans. You will facilitate interdisciplinary team meetings, address medical, behavioral, and community-integration needs, and connect individuals with providers and resources.

Qualifications

• Bachelor's degree in public health, social services, psychology, human services, or a related field.
• Master's degree preferred for advancement.
• Bilingual communication skills are required.
• Knowledge of NJCAT, Person-Centered Planning Tool, NJISP processes, Medicaid requirements, and NJ DDD systems is expected.
• Strong written and verbal communication, organization, and time-management skills.

Responsibilities

• Assess individual needs and complete the Person-Centered Planning Tool.
• Use NJCAT information to determine support needs, goals, and service eligibility.
• Develop, update, and monitor New Jersey Individualized Service Plans.
• Coordinate and facilitate team meetings at various community locations.
• Address needs within each individualized plan across multiple domains.
